Resumo: Female patient, 41 years old, without systemic comorbidities, attended the appointment complaining of an increase in volume in the right mandibular torus. Clinical examination revealed local edema associated with redness and areas of suppuration. With the prescription of antibiotic therapy, there was slight improvement, and occlusal radiography and computed tomography were requested. Computed tomography revealed osteolytic and destructive areas, with a moth-eaten appearance. Given the diagnostic hypotheses of osteosarcoma and osteomyelitis, an incisional biopsy and curettage were performed. Intraoperatively, an extremely osteolytic area with crumbling bone and suppuration was noted, possibly indicating acute infection. The pathological examination confirmed the diagnosis of acute osteomyelitis. After one month, the patient remained healthy without signs of infection. Considering the challenging management of the case, it is important to highlight that due to the moth-eaten radiographic appearance, a biopsy is necessary to rule out the possibility of malignant neoplasms such as osteosarcoma.
