Resumo: Objective: To establish a connection between the development of oral pyogenic granuloma (OPG) in pregnant women and the occurrence of preterm.Study Design: A bibliographic search was conducted in the PubMed, Scielo, LILACS, and Google Scholar databases, using the terms "pyogenic granuloma," "periodontitis," and "preterm birth" in Portuguese, English, and Spanish languages. Results: With a complex etiopathogenesis, oral pyogenic granuloma affects between 0.5% to 9.6% of pregnant women, manifesting in the gums in 75% of cases. Its development is believed to be associated with the exacerbation of pre-existing oral conditions due to the accumulation of bacterial plaque, such as the progression from gingivitis to periodontitis, and the increase in levels of steroidal hormones during pregnancy. The dissemination of periodontal bacteria can reach the placenta, amniotic fluid, and fetal circulation, activating inflammatory mediators and inducing uterine muscle overstimulation and contraction, resulting in preterm birth. Conventional treatment includes oral hygiene support, prophylaxis, and removal of infectious foci, and, only when necessary, surgical excision. Conclusion: OPG is a chronic inflammatory hyperplasia and non neoplastic. During prenatal dental care, individualized treatment prioritizes the control of local irritative factors as a fundamental approach to promote health and reduce the incidence of preterm birth.
