Resumo: Osteonecrosis of the jaw (ONJ) is difficult condition to treat characterized by an exposure of necrotic bone in the oral cavity in patients who have undergone radiotherapy to the head and neck cancer or used bisphosphonates, antiresorptive drugs or antiangiogenic agents. A 55-year-old patient, previously treated for multiple myeloma with bone marrow transplant and using bisphosphonate presented bone exposure in the right lingual region of the mandible. With medication-related osteonecrosis of the jaws as main diagnostic hypothesis, patient began treatment with antibiotic therapy and laser therapy. Five laser therapy sessions were performed: the first Therapy Antimicrobial Photodynamics (aPDT), the second aPDT and removal of spontaneous bone sequestration, third and fourth aPDT and wear of the remaining exposed bone area. Upon return for the 5th session, bone exposure was no more identified and only laser therapy was performed. The patient is being monitored with complete remission and no signs of recurrence.
