Resumo: Ankylosis of the temporomandibular joint (TMJ) is a pathology characterized by the replacement of the normal architecture of the TMJ by fibrous or bone tissue, where there is fusion of the anatomical components of the joint, with trauma being the main cause. Male patient, 14 years old, with a history of trauma from a motorcycle accident, attended the oral and maxillofacial outpatient clinic complaining of limited mouth opening and facial asymmetry. On physical examination, it was possible to observe a maximum interincisal opening (MIO) of 4mm and on imaging examination, bilateral ankylosis of the temporomandibular joints was observed. The proposed treatment was interpositional gap arthroplasty with a temporal muscle flap on the right side and gap arthroplasty on the left side. The patient developed an MIO of 35mm during follow-up 6 months after surgery. The surgical techniques performed are excellent alternatives for treating this pathology in children.
