Resumo: Paracoccidiomycosis is a deep, chronic granulomatous systemic blastomycosis that spreads through blood and lymphatic circulation, affecting internal organs. A 65-year-old male patient is presented with pathognomonic signs and symptoms of mycosis, such as edema of the upper lip, ulcers with a moriform appearance throughout the oral mucosa with an addition of blackish dots and a productive cough, for one year evolution, to confirm the diagnosis an incisional biopsy of the upper lip was performed and sent for anatomopathological examination, the result was papillary hyperplasia with bacterial colonies, without fungal structures, due to the clinical characteristics the biopsy was repeated, with the final result of Paracoccidiomycosis. Is indicated a oral medication, Fluconazole 150 mg, once a week for initial treatment and we refer the patient to a specialist for final treatment. This case demonstrates the importance of clinical diagnosis and its correlation with histology.
