Resumo: Recurrent aphthous stomatitis is the most prevalent ulcerative pathology affecting the oral mucosa. A 50-year-old woman presented a 10-year history of successive episodes of multiple recurrent ulcers in the oral mucosa and tongue region, leading to eating and speech difficulties. Clinical examination revealed multiple, round, painful, major and minor ulcers measuring 5mm in diameter on the lateral border of the tongue, and 1mm on the ventral area, surrounded by an erythematous halo, persisting for five weeks without healing. An excisional biopsy was performed, revealing a fragment of mucosa partially covered by parakeratinized stratified squamous epithelium, presenting a central area of ulceration with exposure of dense connective tissue permeated by an intense mixed inflammatory infiltrate. The diagnosis of recurrent aphthous stomatitis was confirmed based on clinical and histopathological findings. Remission was achieved with the prescription of immunomodulatory and vitamins replacement therapy, improving her quality of life. The patient undergoes periodic follow-up.
