Resumo: Twenty year old male attended the service to do a biopsy in a maxillary posterior lesion, with the diagnostic hypothesis of unicystic ameloblastoma. Tomography showed a well delimited, hypodense lesion, causing cortical expansion and rupture of vestibular, alveolar and palatine bone. An excisional biopsy was performed and histological sections of the material sent for microscopic analysis reveal a benign proliferation of odontogenic epithelium with an ameloblastic appearance, organized in nests and islands, demonstrating a peripheral portion composed of columnar or cuboidal cells, arranged in palisade and central cells loosely distributed, similar to the stellate reticulum of the enamel organ. In between cystic and solid areas of ameloblastomatous epithelial proliferation, numerous ghost cells, sometimes in clusters, and several foci of globose mineralized structures showing dystrophic calcification were observed. Final diagnosis was Dentinogenic Ghost Cell Tumor. Patient was fully recovered after the procedure, being followed for 2 years with no recurrence.
