Resumo: Objective: This narrative review aimed to provide a comprehensive and extensive overview of the current state of the art on this topic and its near-future perspectives related to the diagnosis of osteomyelitis, focusing on the most recent and relevant literature. Study Design: A comprehensive and narrative literature review on the identification and diagnosis of osteomyelitis in radiology was conducted through specific term searches in the PubMed/MEDLINE, Scopus, and Embase databases, evaluating the current state of the art on the topic. Results: Osteomyelitis may present different mixed patterns of osteosclerosis, osteolysis, and bone sequestration. Proper localization of the condition through cone-beam computed tomography (CBCT) is indispensable to determine its extent, prepare for surgery, and enable post-surgical follow-up. CBCT may be recommended to better portray the characteristics and progression of the lesion, with higher potential and predictive value in early diagnosis compared to panoramic radiography. Conclusion: Early identification of subtle radiographic signs demonstrated by CBCT can help identify patients at risk of developing osteonecrotic lesions. It is also worth noting that in addition to imaging tests, histopathology is essential for defining the diagnosis.
