Resumo: Man, 45 years old, leukoederma, denies smoking and alcohol consumption, history of adenomas and intestinal polyps, family medical history of breast cancer (mother) and lung cancer (uncle). Intraoral examination, heterogeneous leukoerythroplastic plaque was observed on right lingual belly. Incisional biopsy was performed using TAT (positive). Histopathological report of microinvasive squamous cell carcinoma. Patient treated by head and neck surgeon, with well-differentiated squamous cell carcinoma as histopathological report. One year of follow-up, a leukoplastic plaque with an erythematous center was observed in same region as the diagnosed oral cancer. New incisional biopsy was performed using TAT (positive), with a histopathological diagnosis of severe epithelial dysplasia and sent for complete removal of the lesion. Histopathological report of epithelial hyperplasia without atypia. This case portrays the importance of monitoring patients diagnosed with oral squamous cell carcinoma and highlights that future investigations should be carried out in patients without a known risk factor.
