Resumo: A 35-year-old woman with complaints of swelling in tongue. Intraoral clinical examination there was a nodule in the dorsum of tongue, of fibrous consistency, normal mucosa color, well-defined limits, and sessile. The initial diagnosis was pyogenic granuloma, inflammatory fibrous hyperplasia and neurilemmoma then was performed an excisional biopsy. Histopathological analysis revealed hyperplastic fibrocollagenous connective tissue with neural structures ranging from round or ovoid pseudo-onion bulb nests to serpiginous or somewhat plexiform arrangements. These findings were consistent with oral pseudoperineuriomas. The patient underwent a two-month postoperative follow-up without any signs of recurrence. Oral pseudoperineurioma is characterized by reactive perineurial proliferation, distinct from intraneural perineurioma. It typically manifests in trauma-prone areas and is marked by nerve perineurium discontinuity, followed by perineurial cell proliferation aimed at protecting Schwann sheath/axons units. Although uncommon, oral pseudoperineurioma should be considered in the histopathological differential diagnosis of traumatic lesions by oral pathologists.
