Título: CHALLENGES IN EVALUATING DEPTH OF INVASION AND TUMOR THICKNESS IN BIOPSIES OF ORAL SQUAMOUS CELL CARCINOMA

Resumo: The assessment of Depth of Invasion (DOI) and Tumor Thickness (TT) in oral squamous cell carcinoma (OSCC) is crucial for staging and appropriate treatment.
Objective: This study aimed to evaluate DOI and TT in OSCC biopsies using digitized slides and to identify the challenges faced in this measurement.
Study Design: Sixty-three OSCC slides from the pathology archive, were analyzed, mostly obtained through incisional biopsies (93.7%). Clinical data of patients were retrieved from medical records, including sex, age, symptoms, and lesion location.
Results: Of the cases analyzed, 84.1% were male and 15.9% female. Mean age was 59.79 (range of 37-89 years). Most patients (68.3%) presented symptoms, with the buccal mucosa being the most commonly affected site. Challenges included 82.5% with positive margins, 31.7% lacking surface epithelium, and processing artifacts observed in 30.2% (inclusion errors) and 15.9% (tissue fragmentation) of cases. DOI and TT were evaluable in only 9 cases, with a median DOI of 1.93mm and TT of 2.20mm. The invasion depth increased by 0.15mm to 0.54mm when comparing DOI and TT.
Conclusion: Significant challenges in DOI and TT measurement in OSCC biopsies were highlighted. In T1 biopsies, differences in DOI and TT measurements are insufficient to change the classification to T2.
