Resumo: Adenomatoid odontogenic tumor (AOT) is a benign epithelial odontogenic tumor that commonly affects young patients. This study aimed to report a rare case of AOT presenting adjacent fibro-osseous reaction. An 18-year-old female patient presented for treatment of an asymptomatic lesion in the maxilla as an incidental finding with unknown evolution time, causing mild facial asymmetry. Cone beam computed tomography showed an unilocular well-defined mixed mass in the left maxilla, involving and displacing the left permanent canine. An excisional biopsy was performed, and the specimen was sent for histopathological exam. Microscopically, the lesion showed a benign proliferation of odontogenic epithelial cells with a nodular arrangement, rosettes of spindled cells, duct-like structures, and dystrophic calcifications. Surprisingly, an adjacent fibro-osseous proliferation was also observed. KRAS mutation leading to p.G12V/R substitution was detected by direct sequencing. The final diagnosis was AOT with a reactive fibro-osseous reaction. The patient is under follow-up with no recurrence.
