Resumo: Lichen planus is a mucocutaneous disorder with different clinical patterns. Bullous lichen planus is a rare entity. A 62-year-old man was referred to the Oral Medicine complaining of painful lesions on his tongue making eating difficult. Stress was associated with reported symptoms. Clinical examination showed on lateral edge of the tongue, radiating white plaques and ulcerations resulting from blisters with liquid content. In the posterior buccal mucosa, a white reticular plaque was observed. An incisional biopsy was performed on the left lateral edge of the tongue, and the diagnosis of lichen planus was made. The histopathological examination showed hyperkeratinized stratified squamous epithelium, showing areas of exocytosis, degeneration of the basal layer and Civatte bodies, intense lymphocytic inflammatory infiltrate, arranged in a subepithelial band. Dexamethasone mouthwash 0.1 mg/ml was prescribed for fifteen days. The lesion regressed after this period, recurring after the mouthwash was discontinued. Support: CAPES.
