Resumo: Congenital granular cell epulis (CGCE) is a rare congenital growth of unknown histogenesis. Usually, CGCE develops on the maxillary alveolar ridge of newborn, and that can lead to dysphagia and dyspnea. We report a case of a 47-day-old girl neonate who presented with one exophytic lesion in the left mandibular alveolar ridge. Intraoral examination revealed a smooth-surfaced, pedunculated mass with normal coloration. Excisional biopsy was performed, and the histopathological findings revealed tumour composed of sheets of round to polyhedral cells with granular cytoplasm and eccentrically placed hyperchromatic nuclei. These findings led to the diagnosis of CGCE. This lesion demonstrates an excellent prognosis, without associated with any syndromes/genetic defects or malignant transformation. Healthcare professionals should be able to recognize and intervene in such rare entities.
