Resumo: Desmoplastic fibroblastoma (DF) is an uncommon benign mesenchymal neoplasm that can affect different regions of the body, rarely occuring in the oral cavity. This study reports a case of an extensive DF in the maxilla of a 60-year-old woman clinically diagnosed as fibrous hyperplasia. On clinical examination, the patient presented an asymptomatic, pedunculated nodule of 4 cm in its greatest extension, on the maxillary alveolar ridge. The lesion had been present for 3 years and was associated with prothetic trauma. Histopathological examination revealed a well-delimited benign mesenchymal neoplasm characterized by proliferation of plump fibroblasts, often multinucleated, dispersed in a collagenous to fibromyxoid stroma. The neoplastic cells were strongly positive for vimentin, focally positive for α-SMA, and negative for S-100 and CD34. Based on these findings, the diagnosis was desmoplastic fibroblastoma. The patient is under follow-up, and currently, after 6 months, has not experienced any recurrences.
