Resumo: The World Health Organization (WHO) in 2022 included new criteria to classification system for oral epithelial dysplasia (OED). Therefore, it is important to assess how these changes influence the prognostic prediction of OED classification. This study compared the diagnosis of OED classification according to the WHO 2022 criteria with the previous criteria to predict the risk of malignant transformation. For this purpose, histological slides from 195 cases of leukoplakia, erythroplakia or leukoerythroplakia with clinical follow-up were evaluated. The cases were graded according to the WHO 2017, 2022 and binary system OED classification. All architectural and cytological criteria present were recorded for each case. The analysis revealed a progressive increase in malignant transformation with the epithelial dysplasia severity, remaining consistent in all classification systems. Multivariable analysis associates alteration in cell shape with increased risk of malignant transformation. The ROC curve established new cutoff points in architectural and cytological alterations to predictive OED malignant transformation. Kaplan-Meier survival curve demonstrated that patients reaching this new cutoff point had a faster progression to cancer. Our findings suggest a new cutoff point for architectural and cytological alterations to predictive malignant transformation considering the additions in WHO 2022.
