Resumo: A 14-year-old female patient presented with complaints of painful ulcers that first appeared on her tongue two months prior. Patient reported having used melaleuca, triamcinolone acetonide, and nystatin with no improvement. Clinically, multiple varying-sized well-defined plaques with ulcerated areas were observed. The lesions were primarily located on the lateral border of the tongue bilaterally and the upper and lower lips mucosa. Diagnostic considerations included syphilis, vitamin B12 deficiency, and major aphthous ulcer. Subsequent testing revealed positive results for VDRL and FTA-ABS, confirming the diagnosis of secondary syphilis. The patient was then referred to an infectious disease specialist for treatment. Syphilis poses a significant public health challenge globally, with increasing incidence rates, particularly among younger populations. The diagnosis of syphilis can be complex due to the overlapping of clinical manifestations with other conditions, in addition to the serious issue of self-medication. Careful evaluation is necessary for accurate diagnosis and treatment.
