Resumo: Amyloidosis is a debilitating disease with systemic or local manifestation characterized by progressive extracellular deposition of insoluble proteins that can affect multiple organs. A 72-year-old male patient, with a history of hyperthyroidism, dysphagia and weight loss, was referred by the physician due to edema and erythema on the tongue. Upon extraoral examination, petechiae near the eyes and hemorrhagic discharge in the right eye were observed. Intraoral examination revealed macroglossia and normochromic nodular lesions on the tongue, as well as petechiae on the palate and buccal mucosa. With clinical suspicion of amyloidosis, an incisional biopsy was performed. Histopathological examination revealed extracellular deposits of homogeneous eosinophilic amorphous material, suggestive of amyloidosis. Diagnosis was confirmed through histochemical examination with Congo red stain. The patient was referred to the medical service for investigation of multifocal organ involvement, being diagnosed with multiple myeloma, and is currently undergoing chemotherapy treatment.
