Resumo: A 61-year-old male was referred for assessment of a right-sided facial swelling of 3-month duration. His medical history included a prior partial mandibulectomy with histopathological diagnosis of ameloblastoma 5 years ago. Clinical examination revealed a significant swelling in the right mandible with spontaneous pain. Computed tomography identified a well-defined hypodense lesion located within the soft tissues of the floor of the mouth, involving the sublingual and submandibular spaces, measuring 11 x 10 x 9.5 cm. Surgical resection of residual mandible and soft tissues was performed. Microscopic analysis demonstrated a solid cellular proliferation characterized by peripheral cells displaying ameloblastic-like differentiation. Additionally, a cribriform arrangement with duct-like spaces, whorled cellular condensations, and dentinoid formation were observed. Nuclear positivity for β-catenin confirmed the diagnosis of adenoid ameloblastoma. Ki-67 was positive in <5% of tumor cells. Currently, the patient is under regular follow-up and no signs of recurrence were observed.
