Resumo: Objectives: Retrospectively evaluate the incidence of chronic periapical periodontitis in patients with head and neck cancer (HNC). Study design: This study was conducted through retrospective analyses of panoramic radiographs of patients diagnosed with head and neck cancer over a 30-year period. The patients periapical condition was assessed by an examiner using the Periapical Index (PAI) score as (1) normal periapical structure, (2) minor alterations in periapical bone structure, (3) structural alterations in periapical bone with mineral loss, (4) well-defined radiolucent areas, and (5) exacerbated periapical lesion. Results: A total of 242 radiographs were analyzed. Approximately 87% of the patients were male, and 78% were diagnosed with Squamous Cell Carcinoma. The most affected region was the mouth, with 53%, followed by the oropharynx with 40%. Approximately 95% presented some periapical alteration. The highest incidence of periapical alterations was in score 2, present in around 88% of the patients, followed by score 4 with 71.1%, and score 3 with 55%. Conclusion: It was concluded that the most patients diagnosed with HNC presented chronic periapical periodontitis scores 2 and 4.
