Resumo: The study reports the case of a 10-year-old man with Noonan Syndrome with a mutation in the SOS1 gene and pulmonary stenosis. At intraoral examination, we observed an expansive lesion in the whole mandible. The initial panoramic radiography showed well-defined multiloculated osteolytic lesions and areas of cortical bone discontinuity affecting the entire jaw. Considering the syndrome the main diagnostic hypothesis was Central Giant Cell Granuloma, confirmed through biopsy and histological examination. The initial therapy was intralesional corticosteroid applications which resulted in progression of the lesion and increased the bone expansion and the tooth mobility in anterior region of the mandible. Therefore, we started the Bleomycin Protocol, performing weekly intralesional applications of Bleomycin (15mg) diluted in saline (5ml) for 5 weeks. After this therapy, the patient progressed with regression and stabilization of the lesions and is on 8 years of follow up with no further complaints.
