Resumo: Primary chronic osteomyelitis is a focal inflammatory disease, devoid of bacterial infection, with local pain and swelling, most common in children. The case pertains to a 7-year-old male patient diagnosed with autism spectrum disorder, presenting with progressive swelling of the left hemiface over a duration of 2 months, devoid of fever or pain. Computed tomography of the face revealed irregular bony expansion with a mixed appearance within the lesion. An incisional biopsy was performed, with diagnostic considerations including infectious osteomyelitis, recurrent mandibular osteomyelitis, or fibrous dysplasia. The histopathological findings were descriptive and non-specific. Following a 3-month interval, an increase in the size of the lesion prompted a repeat biopsy which suggested fibrous dysplasia upon histopathological examination. Subsequent deliberations with the involved medical teams led to the diagnosis of primary mandibular osteomyelitis, prompting the initiation of immunomodulatory therapy under the supervision of the rheumatopediatric team, yielding a favorable response thus far.
