Resumo: Central ossifying fibroma (COF) is a rare neoplasm of odontogenic origin, frequently affects women and primarily affecting the mandible, Usually, these lesions are small and slow growing. We report a case of extensive lesion, painless, involving the mandibular condyle in a 28-year-old woman. Imaging examinations revealed a well-defined osteolytic lesion with subtle radiopaque areas involving the body, ramus and condyle of the right mandible. Main diagnostic hypothesis was a fibro-osseous lesion. Incisional biopsy was performed, and the careful correlation of histopathological, clinical and radiographic findings led to the diagnosis of COF. Due to the significant extent of the lesion and involvement of the condyle, the patient underwent surgical resection. A polymethylmethacrylate (PMMA) reconstruction plate and a prosthesis were installed. The patient has been followed up for 4 months without evidence of recurrence. Despite its rarity, recognizing COFs clinical, radiographic, and histopathological characteristics is essential for differentiation from other fibro-osseous lesions.
