Resumo: Oral cancer (OC) is the sixth most common malignancy worldwide. The most important risk factors for squamous cell carcinoma are tobacco use and alcoholism, which have synergistic effect. The objective of this work is to present a case of OC in a female patient with no history of smoking or alcoholism. An adult white woman was referred by a dentist due to a chronic ulcer in her mouth that was resistant to treatment. The patient was neither a smoker nor an alcoholic, but had a history of high blood pressure treated with antihypertensive medication. Physical examination revealed an extensive ulcer with irregular edges on the right buccal mucosa, suggestive of OC. An incisional biopsy was performed and histopathological examination confirmed the diagnosis using routine stain and immunohistochemistry. The patient was treated at a reference hospital by surgery and radiotherapy. No recurrence or metastasis was observed after four years of follow-up.
