Resumo: Extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue (MALT) is a subtype of B-cell extranodal non-Hodgkins lymphoma that is common in the salivary glands and ocular adnexa. A 59-year-old male presented with a 9-month history of cervical swelling and ocular proptosis, accompanied by pain and ocular movement restriction. Physical and imaginological examination unveiled a mass in the cervical region encompassing the submandibular gland. Additionally, an enlargement of lacrimal glands was noted, leading to ocular proptosis and eyelid masses. An incisional biopsy was performed and histopathological analysis revealed a proliferation of atypical lymphoid cells. Immunohistochemical analysis revealed positivity for CD20, PAX5, Kappa, and Lambda markers, with a lambda predominance, while negativity for CD3, CD5, CD10, CD23, BCL1, BCL6, and AE1/AE3. The Ki-67 proliferative index was around 10%. A final diagnosis of extranodal MALT lymphoma was made. The patient was referred for onco-hematological treatment.
