Resumo: Myofibroma is a benign tumor originating from muscle tissue, characterized by the proliferation of fibroblasts and myofibroblasts, and typically affects infants and young children. We present the case of a 37-year-old female patient, with a 20-year history of heavy smoking, presented with an ulcerated, painful and rapidly growing lesion on the tongue. Incisional biopsy was performed and histological examination revealed spindle-shaped fibroblasts and myofibroblasts in both dense and less cellular areas, accompanied by hyalinized collagen. Immunohistochemistry showed positivity for vimentin, α-SMA, and a low Ki-67 (<5%). These findings led to diagnosis of myofibroma. The patient underwent complete surgical removal and is in follow up. This case reports an atypical clinical presentation of myofibroma, highlighting the critical importance of consistent diagnosis, treatment and follow-up.
