Resumo: A 61-year-old man was evaluated by the dentistry team during hospitalization for diagnostic investigation of a lesion in the oral cavity. He had odynophagia, otalgia, asthenia, loss of appetite and weight loss of 3 kg in the last month. On physical examination, the patient was prostrate and presented with dyspnea upon minimal physical exertion. On oral examination, an extensive ulcerated lesion was found that involved the skin, lip commissure and extended to the left buccal mucosa, with an irregular, ulcerated surface, whitish in color, with ill-defined limits and measuring approximately 3.5 cm in its greatest extent. Laboratory tests were requested which ruled out an association with Human Immunodeficiency Virus (HIV) and an incisional biopsy of the lesion was performed which confirmed the diagnosis of Histoplasmosis. The patient was referred for follow-up with an infectious disease specialist and after 6 weeks of treatment, the oral lesions completely regressed.
