Resumo: QOS is an osteolytic lesion, attributed to disintegrated post-traumatic hematoma, without epithelial lining. We present a 53-year-old female patient, without systemic diseases, with osteolysis in the right mandibular body, sub-acute pain, 4 years of evolution and endodontic molars, aspiration puncture was performed with a luer syringe, extracting bloody liquid, we proceeded to curettage of the bones, without epithelial lining, extracting fibrous tissue and affected teeth with local anesthesia, material is sent for pathological anatomy, we cause bone and mucosal bleeding, filling the bone defect, together with a gelatin sponge, it is sutured with 3.0 polyglycolic acid. Histology shows a wall very focally covered with squamous mucosa, significant acute/chronic inflammation and extensive sectors of ulceration and granulation tissue, necrotic tissue and bacterial colonies. The differential diagnosis is established between a solitary cyst and a cyst of odontogenic origin. By correlating these findings with the clinic, we confirmed the QOS.
