Resumo: Plasmablastic lymphoma (PBL) is a distinct subtype of non-Hodgkin B-cell lymphoma, characterized by a marked predilection to occur in the immunocompromised patients, mostly those infected with the human immunodeficiency virus (HIV). Other peculiarities involve histological aspects, where there is noted monomorphic proliferation of large cells with eccentrically arranged nuclei and round to oval shapes, one or more prominent nucleoli, as well as abundant cytoplasm. Some macrophages may be present in the stroma, resembling, in a lower magnification view, a "starry sky." Furthermore, immunohistochemistry is paramount in differentiating these tumors, where positive staining for CD20 and CD45 can be observed, but greater staining is noted in relation to plasma cell markers CD38 and CD138. The authors aim to present a rare case of high-grade B-cell lymphoma with plasmablastic differentiation with morphological and immunophenotypic characteristics of terminal B-cell differentiation in an HIV-negative patient, highlighting the importance of immunohistochemical panels for histological differentiation.
