Resumo: A 46-year-old woman with facial asymmetry and a prior diagnosis of central giant cell lesion (CGCL) was referred for surgery. She had a history of chronic kidney disease (CKD). Examination revealed nasal deviation and increased volume in the malar region and hard palate on the left. Intraoral examination showed expansion of the vestibular and palatine cortical, crossing the palatal midline. Radiographically, an expansive, predominantly hypodense lesion was observed in the left maxillary region. Laboratory tests (PTH, alkaline phosphatase, phosphorus, and calcium) were requested, which showed alterations suggestive of hyperparathyroidism, thus contributing to the medical teams diagnosis of hyperparathyroidism secondary to CKD. The patient underwent dialysis and partial parathyroidectomy by the medical team. After disease control (PTH: 27 pg/ml) and lesion stabilization with bone formation, a Weber-Fergusson osteoplasty was performed for aesthetic and functional improvement. She has been in post-operative follow-up for 7 months, with her underlying disease under control.
