Resumo: Objective: Oropharyngeal cancer (OPC) incidence rises globally, however, limited data are available in Brazil. We aimed to estimate OPC incidence in Brazil using data from population-based cancer registries (PBCR).Study Design: OPC incidence data by age, sex, and anatomical subsites (CID-O 3) were obtained from PBCRs (2000 to 2020). Crude and age-standardized incidence rates were calculated using the Brazilian census 2010, and the direct method adjusted by the world standard population. Average annual percent change (AAPC) was estimated for trend analysis. Attributable fractions of HPV were calculated according to Latin America references. Results: A total of 17,980 OPC cases were recorded across 30 PBCRs over the entire collection period (ranging from 1 to 25 years). Males (81.58%), aged 55-59 (17.06%), were most affected. Oropharynx (40.58%), base of tongue (24.98%), tonsil (22.52%), and uvula (11.92%) were affected (P < 0.05). Highest incidence rates were in Southeastern and Southern regions, ranging from 3.1 to 9.4/100,000 in males. Incidence trends increased significantly in 10 PBCRs among males and six among females. HPV-attributable fractions for 24% of the population yielded 2,112 male cases and 476 female cases. Conclusion: Incidence trends are increasing in half of Brazils regions for both males and females; nevertheless, comprehensive studies on HPV prevalence in OPC across Brazilian regions are still necessary.
