Resumo: A 48-year-old male patient, Caucasian, was referred to the stomatology clinic complaining of "pain in the tongue". Anamnesis revealed smoking. Extraoral physical examination showed no noteworthy changes. During intraoral examination, an ulcerated, hardened nodule with pain on palpation, measuring 15 mm in its largest diameter, was observed on the lateral edge of the tongue, with a history of growth in the last 6 months, sensitivity to acidic foods. Diagnostic hypotheses were ulcerated fibroma and traumatic ulcer. An excisional biopsy was performed, showing a histopathological report of granular cell tumor with the presence of well-differentiated squamous cell carcinoma. The patient was referred for oncological treatment. Granular cell tumor is a benign lesion derived from Schwann cells, with a predilection for the oral cavity in young adults as a single lesion, commonly on the back of the tongue, with a good prognosis. However, rarely associated with OSCC.
