Resumo: Oral Proliferative Verrucous Leukoplakia (OPVL) is a rare, potentially malignant disorder, multifocal, resistant to treatments with high rate of malignant transformation. Eleven cases of patients with OPVL were selected according to the criteria by Cerero-Lapiedra et al. (2010). A predominance of females (90.9%) was observed, with a mean age of 59.3 years. The most frequent location of the lesions was the alveolar ridge (90.9%). Two cases showed malignant transformation, one at 5 months and the other at 36 months of follow-up. The mean follow-up time was 46 months, and during the last follow-up, evident lesions were present in 10 cases. The diagnosis and management of this condition are challenging, requiring regular monitoring, biopsies to monitor alterations and new lesions. The clinicians must be vigilant about patients with multifocal leukoplakias. The follow-up data are the key to understanding the true nature and progression of this disorder.
