Título: THE CHALLENGE IN THE DIAGNOSTIC OF POTENTIALLY MALIGNANT ORAL DISORDERS WITH LICHENOID INFILTRATE. CASE REPORT.

Resumo: Woman, 74 years old, caucasian, denies smoking or alcohol consumption, hypertensive, with previous diagnosis of leukoplakia. On intraoral examination, a non-detachable, single, non-homogeneous, symptomatic leukoerythroplastic plaque was observed, measuring 3.0cm, with evolution of 10 months, on the right tongue dorsum. Incisional biopsy and histopathological analysis revealed the presence of severe epithelial dysplasia. The patient was referred to a head and neck surgeon for complete removal of the lesion and the report was lichen planus with epithelial atypia by a medical laboratory. When reviewing the report, severe epithelial dysplasia with foci of inflammatory infiltrate with a lichenoid pattern was observed. This report shows challenges in the diagnostic process of potentially malignant oral disorders, especially, when they present with a lichenoid infiltrate. A careful clinical and histopathological analysis is important for the final diagnosis between lichen planus and lichenoid lesion, as it directly affects the treatment and prognosis of the lesion.
