Resumo: Graft-versus-host disease (GVHD) is one of the most common complications in halogen hematopoietic stem cell transplantation patients. This disease may manifest in the oral mucosa and increases the risk for oral cancer. The aim of this study is report a case of GVHD oral manifestation. A 27-year-old male patient, after halogen transplantation due to medullary aplasia was referred to the stomatology department for evaluation of oral lesions. On extraoral examination, alopecia and facial vitiligo were observed. On intraoral examination, areas of epithelial atrophy on the dorsum of the tongue and an asymptomatic non-detachable homogeneous white plaque, measuring 2x2,5cm, were also observed. An incisional biopsy was performed and the histological analysis confirmed the GVHD. The patient was referred to the hematologist for evaluation and treatment. This case emphasizes the importance of a close follow-up by a multidisciplinary team to intercept oral manifestations of GVHD, reducing the risk for oral cancer.
