Resumo: A 54-year-old male smoker presented with ulcerated, burning lesions involving the lips, upper and lower labial vestibule, lower gingiva, and floor of the mouth, which had been noticed for a year. The patient reported a nocturnal cough and weight loss. Physical examination revealed cervical lymphadenopathy and multiple intraoral erythematous ulcerations with a finely granular surface dotted with hemorrhagic dots. The blood count showed anemia with anisocytosis and intense leukocytosis. The sputum bacilloscopic examination was negative for tuberculosis. The lung radiography showed interstitial and alveolar "butterfly wing" opacities. An incisional biopsy with histopathology analysis revealed a chronic inflammatory process rich in epithelioid macrophages, with yeasts inside the giant cells and in the tissues. The Grocott-Gomori silver methenamine (GMS) stain confirmed the diagnosis of paracoccidioidomycosis. The treatment with intravenous amphotericin B was performed, with the remission of oral and systemic lesions. The patient has had no recurrence to date.
