Resumo: A 51-year-old female with a medical history including asthma, hypertension, and psychological disorders, presented with a right maxillary lesion and facial edema, following a recent humeral fracture. Extraoral examination revealed facial asymmetry, while intraoral examination showed vestibular sulcus flattening due to swelling. Initial investigations with a fine-needle aspiration biopsy and panoptic staining indicated a high presence of plasma cells. Further incisional biopsy identified atypical binucleated plasma cells. Imaging, including computed tomography and various radiographs, displayed "punched-out" radiolucent lesions predominantly in the skull and irregular lesions in other areas. Urine and serum analyses confirmed monoclonal gammopathy. These findings led to a diagnosis of multiple myeloma, and the patient was directed towards oncological care. The case underscores the significance of comprehensive diagnostic evaluation following bone fractures, especially in patients with complex medical histories, for the early detection of systemic conditions like multiple myeloma.
