Resumo: A 37-year-old female patient presented to the otorhinolaryngology department with a lesion in the left malar region, with a 2-year history of progressive increase. The patient denied nasal obstruction, epistaxis, an altered sense of smell, secretions, weight loss, drinking, or smoking. Clinical examination revealed an indurated nodule in the left malar region, extending to the nasal ala and jugal mucosa. Computed tomography showed a solid, heterogeneous, and expansile lesion within the skin and subcutaneous tissue of the malar and nasal regions. An incisional biopsy was performed, and histological findings revealed a predominantly dermal cell tumor with monotonous short spindle cells in a storiform pattern and infiltrative growth into adipose tissue. Immunohistochemistry revealed diffuse CD34+ and FXIII- reactivity, supporting the diagnosis of dermatofibrosarcoma. The patient underwent wide-margin surgical resection, neck dissection, and adjuvant radiotherapy. After 15 months of follow-up, no evidence of recurrence was observed.
