Resumo: The objective of this work is to present a clinical case of a vestibular bifurcation cyst detected on a panoramic x-ray during orthodontic follow-up. This is a male patient, 8 years old, asymptomatic, presenting an increase volume in the left mandible region, with expansion of the vestibular cortical region and crepitus in the most superficial region. Radiographically, the lesion was radiolucent, unilocular and well-defined, involving the vestibular bifurcation of tooth 36, which was in the process of eruption. A cone beam computed tomography was requested for better delimitation and surgical management of the excisional biopsy, with the diagnostic hypothesis being a vestibular bifurcation cyst, which was confirmed by histopathology. The tooth was maintained without any complications and with adequate healing, and the patient has been monitored by pediatric dentistry. Knowledge of the etiopathological characteristics of the vestibular bifurcation cyst is important for correct diagnosis and treatment.
