Resumo: This study aims to report the case of a 20-year-old male patient who presented with a complaint of "sore in the mouth" with no relevant medical or dental history. He denied tobacco and alcohol consumption and reported a painful ulcer for 1 month. The extraoral physical examination showed no relevant findings. Intraoral examination revealed white patches on the dorsum of the tongue and an extensive erythroleukoplakia on the lateral border associated with a 1.0 cm ulcer. Histopathological analysis confirmed the diagnosis of squamous cell carcinoma (SCC). The patient was referred for oncological treatment, including excision of the lesion and unilateral lymph node dissection, and remains under follow-up. The emergence of SCC cases in patients under 40-45 years old has garnered attention in the literature. The clinician plays a crucial role in the early diagnosis of these lesions, facilitating a more conservative treatment approach and increasing the chances of cure.
