Resumo: Peripheral ameloblastoma is a variant rare of ameloblastoma, constituting <10% of ameloblastoma cases. It appears on the gingiva and alveolar mucosa, most commonly affects male, is asymptomatic, slow-growing and can be locally invasive. This case report is of a 60-year-old female patient presenting a tumefaction in the right maxillary alveolar ridge, there were no imaging alterations in the bone. The biopsy was performed and histopathological analysis revealed a neoplasm in islands and cords of epithelial, with a central component, similar to the stellate reticulum of the enamel organ, and a peripheral, with hyperchromatic and reverse polarity nuclei and columnar cells in a palisading arrangement. The lesion was connected to the basal layer of the gingival squamous epithelium, suggesting internal proliferation extraosseous, which the final pathologic diagnosis was peripheral ameloblastoma. After a 13 months follow-up, no recurrence was observed. The present case emphasizes histological analysis is fundamental to achieve correct diagnosis.
