Resumo: Paracoccidioidomycosis (PCM) is a fungal disease, endemic in South America, caused by the fungi Paracoccidioides sp. This study presents a case involving a 45-year-old male patient diagnosed with paracoccidioidomycosis. The patient exhibited an increased volume in the right lower lip, accompanied by a painful ulcerated lesion. It was detected two months before his first appointment. He had weight loss, painless, enlarged, solid and fixed lymph nodes in submandibular and neck regions. He was a tobacco smoker for 28 years and a drinker. The clinical findings suggested a neoplasic lesion. Nevertheless, histopathological exam revealed a fungal disease. The patient was referred to an infectologist and had appropriate systemic treatment, after confirming diagnosis by lung CT and lymph nodes biopsy. The oral lesions disappeared after antifungal treatment, observed in follow up after six and twelve months from diagnosis.
