Resumo: Myoepithelial carcinoma arising from a pleomorphic adenoma is a rare neoplasm of the salivary glands, accounting for 3.6% of salivary gland tumors and 12% of salivary gland malignancies. Here, we present the case of a 58-year-old male patient who presented with swelling of the palate. A prior diagnosis of "benign tumor" had been established. Oral examination revealed a nodular lesion on the hard palate measuring 4 cm, firm on palpation and partially purplish, with a significant increase noted over the past 4 months. Radiographic imaging demonstrated involvement of the entire maxillary sinus. Histopathological examination revealed dysplastic clear cell nests and dysplastic epithelioid cell cords. Morphological and immunohistochemical analyses confirmed the diagnosis of myoepithelial carcinoma. The patient was referred for treatment with chemotherapy and radiotherapy at the cancer hospital. This case highlights the critical importance of early diagnosis in the management of malignant lesions, as timely intervention significantly impacts patient outcomes.
