Resumo: Oral lipomas (OL) are benign neoplasms primarily composed of adipose tissue. Objective: This study aimed to characterize the clinical features and histopathological variants of OL. Study design: A narrative literature review was conducted across PUBMED, LILACS, and SciELO databases, encompassing English, Spanish, and Portuguese studies without year-of-publication restrictions. Variables such as gender, lesion size, color, location, and histopathological subtypes were analyzed. Results: 95 papers were included, totaling 502 cases. The majority were reported in Brazil (57.3%). Regarding gender 54.5% were male and 45.4% were female, with an average age of 50.75 years. The average size of the lesions was 2.75 cm, most commonly presenting a yellowish (12.1%) followed by normochromic (8.5%). The buccal mucosa (41.4%) was the most prevalent site, followed by the tongue (18.72%) and lip (11.1%). Information regarding fluctuation in fixation solution was lacking in most cases (98.4%). Histopathologically, conventional lipoma was the most frequent subtype (56.7%), followed by fibrolipoma (22.9%), spindle cell lipoma (7.5%), sialolipoma (5.9%), osteolipoma (2.1%), and angiolipoma (1.5%). Conclusion: Understanding clinical characteristics and that histopathological variants accompanying adipose tissue in OL microscopic composition enhances investigation for clinical and pathologists professionals improving the diagnosis and treatment of patients with OL.
