Resumo: Objective: To investigate the occurrence of subgemmal neurogenic plaques (SNPs) diagnosed in an Oral Pathology service, reporting the clinical and morphological characteristics found. Methodology: Occurrence study descriptive, cross-sectional and retrospective, comprising all cases diagnosed between 1970-2022. Results: Of all diagnoses issued in the referred service, 22 were diagnosed as SNPs (0.12%). Of these cases, 90% affected females, with a mean age of 57 years. Clinically, SNPs presented as papules (36.4%), with a reddish color (50%) and symptomatic (50%), with an average size of 5 mm and a duration until diagnosis of 4 months. Histologically, SNPs exhibited fusiform neural cells in regular bundles (77%), lymphoid tissue (81%), germinal center formation (59%), taste buds (41%), and moderate inflammatory infiltrate (40%), besides the presence of mast cells (77%) and ganglion cells (45%). Surgical excision was the treatment of choice with no history of recurrence. Conclusion: Despite being uncommon, knowledge about SNPs is necessary for oral pathologists and clinicians to avoid overdiagnosis.
