Resumo: Adenomatoid hyperplasia is a rare lesion of the minor salivary glands that presents as a non-neoplastic increase in volume. It is frequently located in the palate, being rare in the upper lip. 63-year-old female was referred for evaluation of a bilateral increase in volume in the upper lip, that had developed over a period of 5 years. Clinical examination revealed an additional fold of bilateral mucosal tissue with a central constriction was observed on the inner aspect of the upper lip. Superficially, the mucosal surface was intact. Another hypothesis was benign double lip. An excisional biopsy was performed, and the histopathological examination revealed fragments of minor salivary gland, ductal structures, and interstitial connective tissue and chronic sialodenitis. Mucosal acini with hyperplasia and hypertrophy are also observed. Ductal ectasia and focal areas of oncocytic duct metaplasia are observed. The final diagnosis was adenomatoid hyperplasia. The patient progressed well and without recurrence.
