Resumo: Lymphoepithelial carcinoma (LEC) rarely occurs in the oral cavity, an association with Epstein-Barr virus (EBV) is commonly observed in nasopharyngeal lesions. A biopsy of a lingual apex nodule was sent from an 85-year-old female. Clinically, the lesion was described as oval, sessile, firm, and normochromic. Histopathological analysis showed a malignant neoplasm composed of epidermoid cells isolated or arranged in islands and cords, exhibiting a syncytial appearance. The cells were pleomorphic, with vesiculated nuclei containing acidophilic macronucleoli. The stroma was markedly lymphoid with few eosinophils. It penetrated the tumor proliferation, making it difficult to visualize the islands and cords of epidermoid cells. Tumor cells were immunostained for anti-CKPool, anti-EMA, and anti-EBV, suggesting a diagnosis of LEC. The patient was referred to the Head and Neck Service for oncologic treatment. A combination of histopathology and immunohistochemistry is essential for the diagnosis of LEC. Oral lesions may also be associated with EBV.
