Resumo: Pilomatricoma represents 1% of benign skin tumors and originates in the hair follicle. It affects children and young adults. The diagnosis is clinical, presenting as solitary subcutaneous tumors with slow growth, painless, without itching or symptoms, well delimited. The skin over the neoplasm may appear normal, pale, red, or bluish. The lesion is fixed to the skin covering it and movable in relation to deep planes. Ultrasonography aids in the diagnosis, showing a calcified or hypoechoic nodule. Histopathologically, we see islands of cells in a circular pattern, with nucleated basaloid cells located at the periphery and anucleated ghost cells in the center. A 47-year-old woman presented with a small, hard, well-defined, bluish nodule in the right masseteric region, painless. Upon removal, it was well adhered to the dermis. Histologically, islands of nucleated basaloid cells surrounding central anucleated shadow cells were observed, closing the diagnosis of pilomatricoma.
