Resumo: Hepatocellular carcinoma (HCC) is the sixth most common cancer in elderly patient with very rare presentation in young population. Here, we present a case of a 28-year-old who presented a painful swelling associated with paresthesia in mandible, lasting two weeks. Her medical history showed a metastatic unresectable hepatocellular carcinoma. The physical examination showed a submucous nodule, measuring 2 cm associated with the canine and mandibular premolars. On tomographic examination, an osteolytic lesion with rupture of the vestibular cortical was observed. Oral metastasis was the main diagnostic hypothesis. After the Multidisciplinary team (MDT) meetings, oral biopsy was not indicated due to severe hepatic encephalopathy. Treatment consisted of radiotherapy for algic control. After five months, the patient died of brain damage. Although rare, this case emphasizes the agressiveness of HCC in young patients with very poor prognosis and the importance of a MDT in cancer treatment, to discuss appropriate management.
