Resumo: The lateral periodontal cyst is considered an odontogenic cyst, with unknown etiology. Not a frequent diagnosis. Its preferred location is the lower premolar region. This lesion is characterized by a slight increase in volume in the periradicular region of a vital element, commonly translucent or slightly purplish in color. Some cases, can cause expansion of the cortical bone. Radiographically, a radiolucent, unilocular and well-defined lesion is observed. Female patient, melanoderma, 55 years old, who sought care complaining of increased volume associated with discomfort in the region of element 24, radiographically noted a discreet radiolucent area, with rupture of the vestibular cortex. After a diagnostic process, including physical examination, cone beam tomography and incisional biopsy. The microscopic examination revealed that it was a large lateral periodontal cyst, according to clinical hypotheses. The patient underwent surgical excision and was advised to maintain clinical follow-up. The operated area remains in excellent clinical conditions.
